HEALTH COVERAGE & RATES FOR 2003

The following outlines the health coverage in force for the Township for 2003. PPO major medical is offered through Medical Mutual of Ohio with renewal in August. The vision plan is carried by VSP (Vision Service Plan), also with renewal in August. The dental plan is with Delta Dental with renewal in January. There is no change in suppliers or coverage level from 2002. Following are the current rates:

Family Couple Single

Medical Mutual $ 888.16 $ 628.80 $ 314.40

Vision Service Plan $ 25.09 $ 25.09 $ 11.08

Delta Dental $ 50.41 $ 50.41 $ 19.44

We will continue with our current policy of offering all employees and elected official health coverage. Fulltime employees and elected officials are required to pay a monthly copay if they choose to take the major medical. Part time employees are eligible for coverage at their own expense.
