CAFETERIA PLAN BENEFITS
WHEREAS,
the Board of Township Trustees has elected to contract for group health care
services with health insuring corporation holding certificates of authority
under Chapter 1751, Section 505.60 and 505.61 of the Ohio Revised Code for
township officers and employees,
WHEREAS,
the Township provides uniform coverage under any such contracts for township
officers and full-time employees and their immediate dependents. Part-time
township employees and their immediate dependents are offered the same uniform
coverage, but costs associated with this coverage are not met by the township,
WHEREAS,
The township may procure and pay all or any part of the cost of insurance
policies that may provide benefits for hospitalization, surgical care, major
medical care, dental care, eye care, medical care, hearing aids, prescription
drugs, life insurance or any combination of the foregoing,
WHEREAS,
any township officer or employee may refuse to accept the insurance coverage
without affecting the availability of such insurance coverage to other township
officers and employees,
WHEREAS,
effective September 5, 2000 in Ohio Revised Code, Section 505.603, in lieu
of providing benefits to township officers and employees under section 505.60,
505.601 or 505.602 of the Ohio Revised Code, the board of township trustees may
offer benefits to officer and employees through a cafeteria plan that meets the
requirements of section 125 of the “Internal Revenue Code of 1986” 100
Stat.2085, 26U.S.C.A. 125, as amended.
NOW, THEREFORE,
the Board of Trustees of Richfield Township, Summit County, State of Ohio,
by this resolution agree to a policy authorizing an officer or employee to
receive a cash payment in lieu of a benefit otherwise offered to township
officers or employees, but only if the cash payment does not exceed twenty-five
(25%) percent of the cost of premiums or payments that otherwise would be paid
by the board for benefits for the officer or employee under an offered plan.
FURTHER RESOLVED,
that no cash payment in lieu of a benefit shall be made unless the officer
or employee signs a statement affirming that the officer or employee is covered
under another health insurance/health care policy, and setting forth the name of
the employer, if any, that sponsors the coverage, the name of the carrier that
provides the coverage, and an identifying number of the applicable policy.
